It takes time to build a better, stronger version of yourself. In the throes of perimenopauses chaos, it's sometimes hard to see beyond the symptoms you're suffering and believe that restoring your vitality is actually possible. I understand that more than you know. It's the reason why I'm so passionate about sharing the hierarchy of healing. Offering an alternative pathway to help every woman feel better by restoring her foundational health so she can live her life fully again. How could I not, when every woman deserves to live as the most vibrant version of herself? I knew I'd found a path to healing when I saw my own biochemistry shift in my twenties, years before perimenopause, to the point where I had that natural energetic waking in the morning, something I thought would never happen for me due to my health issues. I regained a level of vitality I never believed I could feel again. Strangely enough, this all happened just before I fell pregnant with my son. I still get emotional as I write this because, as you'll learn more about in the next chapter, as a young girl, I was fat-shamed by my vaccination nurse in my first year of high school. However, this difficult moment ignited my curiosity about my weight and prompted me to explore the possible root causes beneath the symptoms. I started to ask why I was different from my friends, who were thin and beautiful, if our behaviors were the same.

From Devastation To Determination

But the true origin of my healing journey began when I was 24, on a day that I will never forget. The day my doctor informed me that in no uncertain terms I was infertile. You can imagine the devastation I felt being told at such a young age that I would never be able to have a child. Yet, as I would later discover, the so-called solutions my doctor prescribed weren't solutions at all. In fact, due to his prescriptions, some of my health issues worsened. That moment marked a turning point. I became serious about researching the tools and the treatment my body truly needed to heal, healing my gut, rebalancing my hormones, and reclaiming my metabolic health for good. By 29, I felt pregnant naturally. I felt like the most vibrant version of myself, living a life filled with joy I had never known before. That same happiness, that same joy de vivre is what every woman deserves. Imagine you naturally wake without an alarm. You love Pilates, yoga, boxing, walking, riding your bike, or whatever movement fills your heart. You head out of the door to take that 5 a.m. class or hit the trail for the run. You open your eyes every morning with a fire in your belly rather than relentless exhaustion because you're once again synced to your natural circadian rhythm. We will talk more about this later on. With your cortisol rising naturally with the sun, you literally jump out of bed fully energized and planning your day. Every morning you ask yourself, what do I want to do today with this one precious life I've been given? Since you're no longer in a deficient state, which will be discussed more later in the book, you've nourished your body to work with your biochemistry, you're able to trust that your body will support you in what you want to do. As the sun sets at the end of the day, you settle into your evening routine. You pay attention to your body's tired signals, allowing it to slow down and quiet enough so that you can hear what it needs. Then you don't deprive your body, mind or soul of anything. You nourish it with the right foods, nutrients, joy, rest, vitamins and connection it needs. Finally, you drift effortlessly off to sleep, the kind of deep restorative sleep your body craves. If you're asking is this really possible? Without a single doubt it is. When you nourish your body according to the hierarchy of healing and implement the individualized precision medicine my protocol offers, your body can heal in a way that it needs to and is sustainable. As I will share in part two, your vitality is achieved when you do the work to repair your gut health, rebalance your hormones, and reclaim your metabolic health. You'll see that each phase in the hierarchy of healing works together to heal your body and reset your biochemistry, which perimenopause has put offline. When that happens, you begin to reclaim your vitality, which lies at the very center of my protocol.

The Three-Phase Healing Hierarchy

The hierarchy of healing. The hierarchy of healing is my three-phase treatment protocol that addresses the key underlying contributors women experience during perimenopause and beyond. These three elements don't work in isolation, they're deeply interconnected, which is why I've represented them as overlapping circles in the images. When you repair your gut and immunity, it becomes easier to rebalance your hormones. When your hormones are balanced, reclaiming your metabolic health becomes achievable, and when all three are working together, you experience true vitality, not just the absence of symptoms, but genuine energy, clarity and well-being. This is what extending your health span looks like in practice. How to work with your biochemistry, not against it. This may sound strange, but my goal with every patient is that she eventually no longer needs me. I want her to become such an expert in her own body that she learns to implement the vitality protocol herself, mastering how to eat, sleep, move, think, test, prescribe, and connect in ways that support her unique biochemistry. I want her to build her own therapeutic dispensary filled with clinical grade medicines her body needs, understand exactly which foods nourish her optimally, know how to manage stress, move in a hormone-supported way, develop relationships that serve her well-being, and interpret her body's signals through proper testing and tracking. This is the end point, or really her entry point, into living her fullest life. When her gut is healed, her hormones are balanced and no longer wreaking havoc. Her metabolism is flexible and responsive, and she has achieved what I call unconscious competence. She has become completely autonomous in her health, the expert in her own body and biochemistry. You might be thinking, but the starting point to healing looks very different for each person. Yes, you're correct.

Testing Root Causes Like Insulin Resistance

For each woman who walks into my clinic, we begin with a thorough assessment to identify her symptoms and health issues, followed by a medical history to determine her starting point. As her practitioner, what I'm looking for is the root causes beneath the symptoms and issues. We then do the proper testing called the full foundational blood panels to properly diagnose any health challenges that may have been missed. To name a few issues that can be detected through these blood panels, it might be an iron deficiency, an imbalance in her thyroid, or perhaps insulin resistance. This testing can identify what hormone levels are imbalanced. With that knowledge, we now have the critical information we need to move toward resolving her symptoms according to the hierarchy of healing. Every woman's body and its biochemistry are unique, and so is her journey to healing. It's important to understand what insulin resistance is. Insulin resistance is when your cells stop responding properly to insulin, making it harder to use glucose for energy. During perimenopause, this creates a perfect storm. Your brain's ability to use glucose as its primary fuel can decline, while inflammation and blood sugar instability often increase, leaving many women with less energy than ever before. The result? Stubborn weight gain around the middle, brain fog, exhausting energy crashes, and intense sugar cravings that feel impossible to ignore. As each patient journeys through the hierarchy of healing, over time they become so attuned to their bodies, their natural rhythms, what makes them feel good and what doesn't, that they begin to know what their body is lacking and what it needs. The protocols they learn in the hierarchy of healing teach them expertise in their own bodies, or what I call body literacy. When women achieve body literacy, they are no longer working against their biochemistry because they now have gained the knowledge and tools to work with it and support it. When you have a high level of body literacy, you become unconsciously competent and as I mentioned completely autonomous in your health. You intuitively know how to eat in ways that nourish your unique biochemistry and support hormonal balance, so you will never need restrictive diets again. You understand exactly how to sleep to support your natural circadian rhythms and allow deep restoration. You move your body in ways that energize rather than exhaust, knowing precisely what types of movements serve your hormone cycles and current life phase. The way you respond to stress will be transformed and the relationships in your life will become more intentional, allowing you to connect with people who truly nourish your well-being. This level of unconscious competence means you're thriving rather than just surviving, working with your biochemistry rather than against it and completely in control of your health destiny. Some of these terms may sound unfamiliar right now, but as you listen further, you will learn what each concept means and the role it plays in your health and overall well-being. For now, I want you to meet a few women who have successfully implemented the hierarchy of healing and the vitality protocol, restoring peace and well-being in their bodies and lives. These are not their full medical histories as I don't want to overwhelm you. Each one is a real woman's health journey from the moment she walked into my clinic, the issues she reported and how they were resolved, and what her life looks like now. I'm intentionally speaking with my doctor's hat on and also through my heart. These are real women's journeys. They're not full technical case studies because I want you to connect with their stories and not get lost in the medical jargon. Renee’s Gut Repair Changes Everything

Renee When Renee walked into my clinic I saw a beautiful mum in her early forties who clearly looked after herself. However, I quickly discovered that she held a devastating secret just below the surface. It's a struggle she didn't want the world to know about, including her children or her partner. It affected the pain she endured daily, her mood and the relationships she held most dear. Serious and chronic issues disrupted Renee's daily life. Renee reported the following symptoms Severe gastrointestinal issues, including severe bloating and constipation, extreme anxiety, mood swings and mum aggro, low mood. These are just a list of symptoms on paper. What you can't see is that the severity of her gastric issues had gotten to the point where she was not able to have a bowel motion by herself. She turned to weekly enemas that, as you can imagine, were time consuming, messy and complicated. Or she faced the opposite problem where she couldn't control her bowel motions. According to Renee, the one good thing about her symptoms was that they waited until the end of the day to worsen. So for the most part, she was able to run her kids to school and head into work without incident. But by the time she returned home, Renee had extreme bloating from her belly button to the top of her pubic bone, to the extent that she looked six months pregnant. You can imagine the devastation and shame she felt and the impact of that shame on every area of her life. In addition to the severe pain she suffered daily, Renee was most concerned with its effect on her relationship with her husband. Ashamed of what she looked like, she didn't want him to see her or touch her. Early on, Renee was intensely skeptical that I could help her because so many other doctors had failed to resolve her symptoms. Her GP had run the blood tests he felt he needed, and told her in typical fashion, everything looks fine, your results are in normal range, and there's nothing wrong with you. Other practitioners told Renee, you're just anxious, you need to slow down and lower your stress. One naturopath had done deeper testing on her gastrointestinal tract which showed results of a candida overgrowth, blastocystis homnis, and streptococcus, all of which indicate that some serious gut pathogens had set up camp, and these bad guys were running the show. To treat her conditions, the practitioner prescribed antimicrobials to kill the pathogens. It was the correct decision to treat the pathogens, but in Renee's case, the antimicrobials made her symptoms worse. Her symptoms worsened because the treatment did not follow the hierarchy of healing in the right order. The practitioner didn't first heal, seal, and repair Renee's gut. Chapter 7 will delve deeper into why you always begin with healing the gut. For now, all you need to know is that it wasn't the prescription that failed Renee, it was the fact that her underlying issues weren't addressed first, so the antimicrobials could be effective. When she presented with a leaky gut, her secretory IgA or Sega on stool testing was almost non-existent, which meant her frontline immune defense in the gut was very weak and less able to cope with a strong course of antimicrobials. Secretary IgA is concentrated in the protective mucus layer that lines your intestines and acts like a shield, helping to trap and neutralize pathogens, the bad guys, so they can't easily penetrate the intestinal barrier and set up camp. It's important to understand what secretory IgA is. Secretary IgA is your gut security system, an antibody that helps capture harmful bacteria and toxins before they get through your gut lining and deeper into your body. Because around 70% to 80% of your immune cells and roughly 90% of your body's serotonin is found in the gut, healthy secretary IgA levels are crucial for robust immunity and for supporting the gut brain pathways that influence mood. During perimenopause, chronic stress and hormonal shifts can contribute to lower secretary IgA in some women, leaving you more vulnerable to digestive issues, frequent infections, food sensitivities, and the anxiety or low mood that often shows up when your gut and its feel-good signaling system are under strain. The gut is where your immune system rides and where 90% of your serotonin, the feel-good hormone, is produced. Since Renee's gut was not repaired first, meaning her thick, healthy secretary IGA was not fully restored, she basically had holes in her gut. Her body couldn't tolerate the antimicrobials, making her symptoms worse. By the time she saw me, Renee was completely hopeless that a solution existed to fix what was wrong with her. I assured her that wasn't the case at all. Renee's treatment. My prescription for Renee was based on a simple formula. I first focused on her digestion and detoxification pathways to help with elimination, and then we repaired her gastrointestinal tract. We began with a gentle repair and detox with Sacromyces balladi and lactoferin, all prescriptions that help heal and seal the gut, increase immunity and gently detoxify. We focused on digestion, gut repair, and detoxification because a huge issue for Renee was endotoxemia, where bacterial toxins from the gut called endotoxins leak into the bloodstream. Triggered by the fact that she wasn't able to detoxify through elimination, leaving a toxic load recirculating in her bloodstream. Not a good thing. To address her extreme constipation, we worked on boosting her digestion so she could move her bowels properly. We also worked to tone down her anxiety, which meant taking her sympathetic dominant nervous system, her fight or flight state, off overdrive by regulating her cortisol levels. The stress hormone also responsible for punching holes in her gut. In a matter of weeks, Renee was able to move her bowels by herself, no more messy enemas required, and the severe bloating was gone. She was sleeping deeply because, as her gut was repairing and her secretary IgA and microbiome recovered, her gut began to produce and regulate serotonin more effectively. Because serotonin is the building block for melatonin, the hormone that tells your body it's time to sleep, this helped restore her natural sleep wake rhythm. Renee said, I not only feel physically better, but everything in my life just feels easier. What do you mean exactly? I asked. She answered, Nothing in my life has really changed between work and my kids, the constant stress and high output, but I no longer feel overwhelmed. It's like my life feels more manageable. Renee even considered applying for a new job, something she wouldn't have ever considered when we first began due to the severity of her issues. She later applied and got the job. Renee was a woman who needed a broader, more specialized approach, a bigger toolbox than what her GP or naturopath was able to offer at the time. Rather than concluding that nothing was wrong, each practitioner could have supported her further by acknowledging the limits of their own testing and treatments, and by referring her to someone with deeper experience with the issues Renee was facing. As clinicians, one of the greatest gifts we can offer our patients is knowing when our toolbox is not enough, and being willing to reach out to colleagues whose skills complement our own.

A Bigger Toolbox With Multiple Lenses

What's in my toolbox? Someone once asked me to describe how I approach women's health. Basically they were asking what my toolbox includes. I recently read a passage from Shane Parrish's book, The Great Mental Models, and he might describe it best. Every different way we look at things, the filters we use, will give us varying information that leads us to new discoveries. When we look through just one filter, which he refers to as a lens, we can only see one result. But when we look through multiple filters, we get additional information as well. This concept could help us as practitioners see results that we wouldn't have noticed otherwise. Similarly, my toolbox, my comprehensive approach to woman's health, is built with multiple lenses to see each woman's unique issues and challenges. I look through the lens of a functional medical practitioner and a naturopath, combining the precision of advanced testing with the wisdom of natural therapeutics. I apply the lens of a trauma-informed coach and mentor, understanding that a woman's health is intrinsically linked with her experiences and trauma. I will speak more about this important component later. I integrate both Western and Eastern medical understanding, drawing from nearly three decades of clinical experience. This way I'm able to offer each woman multiple lenses through which to see her unique issues. I can offer a bigger toolbox to help her heal and reclaim her vitality and vibrance. Renee required a big toolbox with all of these lenses working together, beginning with healing, sealing, and repairing her gastrointestinal tract. In the next case study you'll meet Charlotte. While her circumstances and symptoms differ from Renee's, she too needed a bigger toolbox, one that went beyond just her biochemistry. Charlotte's journey is why I believe a woman's health is not just biochemical, it's also psychoemotional and social.

Charlotte’s Boundaries Sleep And Fertility

Charlotte Charlotte came into my clinic presenting as perimenopausal. She was in her late thirties, a nurse who worked the night shift and newly married to the love of her life. They hadn't yet started a family, but falling pregnant was top of mind for both of them. Charlotte was tiny in stature but big in her heart and Filipino. Her ethnic background was important because she was genetically more inclined towards insulin resistance, in addition to her night shift work, stress and circadian rhythm disruption. As I walk you through her journey, I want to point out that Charlotte is a perfect example of why health is biopsychosocial. In other words, it was not just her biochemistry that lay at the root of her issues. Her emotional state and her environment were also working against her. When Charlotte came to see me, she was taking five. Five different pharmaceutical medications. Five. The medications were to deal with anxiety, reflux, antinausea, and to help with her gastrointestinal tract. For someone so young, I believe she shouldn't be on so many medications. In terms of her symptoms, Charlotte presented with the following insulin resistance, gastrointestinal dysbiosis and reflux, again the bad pathogens were running the show, a severe circadian rhythm disruption from her night shift, bloating and irregular bowel motions, polycystic ovarian syndrome, that I know so well, causing irregular ovulation, irregular periods, resulting in weight gain and for Charlotte issues with fertility. During our initial session it became clear that Charlotte struggled to draw strong boundaries with her new and very domineering mother-in-law, who wanted to continue exerting control over her son, a strong tenant of Filipino culture, according to Charlotte. She understood that her husband was in a tricky place trying to please the two women in his life, his mother and his wife. But she was underestimating the additional stress this placed on her nervous system, which then wreaked havoc on her gastrointestinal tract. What did her doctors tell her? You just need to chill a bit. Reduce the stress in your life. You have a bit of reflux, but you can take medication for that. My question for Charlotte was for the rest of your life? That is not an acceptable option in my book, when there are others available that treat the root cause for good.

As we began building her treatment protocol, with her intelligence and medical background, Charlotte quickly embraced the hierarchy of healing. She understood that the cortisol, the stress hormone, resulting from her anxiety around her mother-in-law, had the capacity to punch holes in her gastrointestinal tract. She was completely mindful that she was overthinking, worrying, and ruminating on the situation. One of the first things I did was give her permission to draw strong personal boundaries around herself and her relationship with her husband, to express what it was she needed in order to feel safe. She looked at me in disbelief, she had never been given permission to do this before. For her physical symptoms, we began by healing, sealing, and repairing her gut. We got Charlotte off her medications. To treat her dysbiosis, an imbalance in the gut microbiome where harmful bacteria outnumber beneficial ones, I put her on an insulin-resistant food protocol so that her insulin levels would balance out. Within three weeks, her levels were within normal range, which then reversed her prediabetes. The next step was to balance her hormones. We identified that Charlotte's night shift work was massively disrupting her natural circadian rhythm, which directly impacted her ability to ovulate regularly. Then we got her progesterone levels up so that she could successfully ovulate. We will talk in greater depth in chapter 6 about progesterone's critical role in rebalancing your hormones. A few weeks later, her OBGYN confirmed she'd successfully ovulated, and the doctor couldn't believe how quickly it had happened. Although Charlotte didn't fall pregnant in the following month, I had no doubt she would very soon. In this case study, I want to circle back to the biopsychosocial element in healing. Yes, Charlotte's disrupted circadian rhythm played a role in her ovulation, but so did the stress of trying to please an overpowering mother-in-law who lives with her and her husband. The human body is less likely to fall pregnant if it believes you're in a war zone. You will not ovulate and you will not be able to procreate because the messages your body is receiving tell it you are in a state of war. And what comes after war? Famine. So both Charlotte's night shift work and the stress of her mother-in-law only reinforce this message to her body. Until things shifted, she would not fall pregnant. Charlotte desperately wanted her own nuclear family living alone in their home. In Charlotte's life, there was a cultural disconnect between the traditional Filipino family structure and our modern world. Her low progesterone levels and her POS impacted her ovulation, but it was also the stress of her needs not being met with proper boundaries being set, and knowing it's okay to want and ask to live the precious life she desired. It's okay and vital to ask for what she needed to feel safe, for her body to feel safe and to protect her biochemistry. Once Charlotte's body felt at peace, as the OBGYN scan showed she was able to ovulate. At the time of writing this book, I predict that soon Charlotte will fall pregnant with the child she and her husband were wishing for, and the grandchild her Filipino mother-in-law wanted so desperately, and yet whose behavior in a sense was preventing it. Charlotte is a case in point of why I believe you can't heal someone without helping them understand the social and psychological components. It's not just about healing their biochemistry. Health is all those things, biochemical, psychoemotional, and social. I firmly believe that we, as doctors, need to speak to our patients about their lives beyond symptoms. Otherwise, we cannot get to the root causes of their issues.

Your Symptoms Matter And So Does Story
